Fill out the ESC Provider Information form and send it to us.  After we review it and check your references, we will let you know if you are approved.

Many organizations applying for ESC assistance already have a consultant in mind that they would like to work with. If you are an approved provider, we encourage you to refer ESC-eligible organizations to us. If you intend to work with a group who has applied for ESC assistance chances are you'll be hearing from us. We maintain an active internal database of approximately 500 consultants and trainers throughout the country.  This database is used to match organizations who request a provider with one from their area who fits their needs.
